For your issue, if you are new and do not know how to create Sub types and anything on Delegation, I would advise you to go by Rick's advise of adding additional variables to the decision item.
1) Copy the standard Decision task to a custom task in PFTC
2) In the task container, add the necessary container elements which you need to display in the body
3) In the task Description (the body), place the necessary text and the container elemnts for values at runtime
4) Ensure you have these values in the workflow container
5) In the workflow, call this custom Decision task
6) Pass the additional variables from workflow to task in binding
